The form is convenient to use but hard for employees to understand sometimes. Our off site employees receive through the mail and have to read on their own. The second and third violations are hard for them to follow. NOt sure how you can make a three tiered disciplinary form less cluttered. ***Customer has been contacted and satisfied by Amsterdam
